We do not know for sure, because there are two ways this happens. One is that God gives the saint the ability to hear your vocal, auricular prayers. Saints cannot read your mind; only God can read your mind. So say you prayed silently: in that case you prayed to God in the name of a saint, and God reads your thoughts and tells that saint, you have someone praying to you; you do not know it, but here is the prayer. And that saint will follow up on whatever he has to do. So no prayer you ever say is wasted. Do not think nobody is hearing you: God hears every prayer, and he will give it to the appropriate saint as you have asked. That is what happens when you are praying in your mind.
